How to collect patient feedback with online questionnaires?

The importance of collecting patient feedback is evident – it helps you to learn patients’ satisfaction and obtain suggestions about improving your service. With the help of well-designed questionnaires, collecting patient feedback turns into an effective tool for enhancing your clinic’s success. However, how to do that without overwhelm your patients and without increasing the tedious paperwork. The answer is simple – online patient feedback questionnaires.

Online questionnaires are easy to create, beautifully designed and can be accessed directly from the browser. Metaforms provides a secure way to collect and analyze patient feedback with no coding skills required. Moreover, it can be integrated with your existing systems or mobile apps. All forms are mobile friendly and make it easy for your patients to fill out directly from their mobile device.

How to create patient feedback questionnaire with Metaforms:

Create Data Model

First step is to create a dataset or so called data model. Datasets represent groups of data points that help collect and analyze data. Datasets can consist of unlimited fields of different types such as string, date, number, etc. For each field that we will need later in our feedback questionnaire, we need to create a corresponding data field and choose its data type.

Building forms

After the dataset is created the next step for creating our patient feedback questionnaire and capturing data is to create a question. The questions are ‘mini forms’ used to group fields logically. They can be placed on a form page and also reused across multiple forms if the forms look similar. Each input field in a question has to be linked to a dataset field, this way we collect data.

You can completely customize the form’s layout with a wild range of elements/controls such as text areas for open questions, check boxes for multiple choice answers, date and signature fields, image and file upload, etc. You can also define whether the fields are visible or editable by users, and create rules and dependencies for each field.

When we have a form, we can use it in many different scenarios, or “projects”.

Create a Project

Next step will be to create a project, which allows us to use the form we have created.

During creation, we need to choose the dataset and which form (or forms) we are going to use in this data capture scenario. Once it is created, we can use it to assign the form to the groups or users of our choosing.

Choose Assignments

You can assign each active form of a project to specific users or user groups and collect responses from one or multiple questionnaires. You can create anonymous group to enable anonymous data collection, anonymous surveys, etc.

Fill in forms

Metaforms Navigator is the tool used by the assignees to fill in data and manage their forms. A user can save the form at any step of the progress. Data from the forms is sent to the project owners allowing them to analyze it.

 Analyze the collected data

You can access data analysis from both Metaforms Manager and Metaforms Navigator. The collected data is displayed in tables that are composed of fields selected from the dataset, specified in data views. A list of all surveys sent to users is available for the project, granting you an easy way to check the status of each form. Also, answers of each survey can be viewed in details, independently of its status. You can customize the data analysis to suit your business needs by filtering the results based on certain questions.

With Metaforms you can build beautifully designed online patient questionnaires from scratch in minutes or you can get a head start and choose from our customizable form examples.
