1. Create Data Model
First step is to create a dataset or so called data model. Datasets represent groups of data points that help collect and analyze data. Datasets can consist of unlimited fields of different types such as string, date, number, etc. For each field that we will need later in our feedback questionnaire, we need to create a corresponding data field and choose its data type.
2. Building forms
After the dataset is created the next step for creating our patient feedback questionnaire and capturing data is to create a question. The questions are ‘mini forms’ used to group fields logically. 5. Fill in forms

6. Analyze the collected data
You can access data analysis from both Metaforms Manager and Metaforms Navigator. The collected data is displayed in tables that are composed of fields selected from the dataset, specified in data views. A list of all surveys sent to users is available for the project, granting you an easy way to check the status of each form. Also, answers of each survey can be viewed in details, independently of its status.
